- Professional dental cleanings & fluoride (2 per benefit year)
- Perio maintenance cleanings (3 per benefit year) covered at 80%
- Radiographs as needed (X-rays)
- Doctor examinations & Diagnosis (2 per benefit year)
- Emergency Exam as needed
- Cosmetic & Dental Implants Consultations as needed
- 20% discount on all services
Notice to patients with dental benefits (Insurance)
- Dental eligibility is not a guarantee of coverage as actual benefit payments are determined only when a claim is adjudicated.
- Major categories are commonly used and represent a grouping of related procedures. Additional procedures in this category may or may not be available under your plan.
- Most dental benefit plans contain certain exclusions and limitations. The patient should refer to his/her plan booklet for complete plan details, exclusions & limitations.
- Your dental insurance plan is likely to determines that a less costly service than the one you requested therefore they will benefits based upon the less costly service.
- Initial installation of fixed or removable bridgework/dentures is only available to replace one or more natural teeth lost while Dental Expense Benefits for the covered person are in effect.
- Insurance Deductibles & Co-Payments
You will need to pay any deductible and fee co-payments the day of your appointment with cash, check, or Credit Card. If your insurance company has denied or not paid your account in full the balance of your account will then become your responsibility.
We accept all major credit cards, Cash or Checks
If patients wish to use their dental insurance, they need to call our office and give our insurance secretary their insurance information so that she can find out what their insurance provider will cover prior to their first visit
We will gladly help fill out all those complicated forms for you and even accept the assignment of insurance benefits so that your “out of pocket” can be minimized. We will even allow our office insurance “expert” to evaluate your particular insurance plan and help you maximize your reimbursement.

We cannot bill your insurance unless you bring all insurance information. Should you have any change in your insurance information please inform the practice before your appointment or any services are rendered.

Our doctors provide dental services to you not your employer or dental insurance company; therefore you are ultimately responsible for the payment of all fees for our service.
